After providing you with a thorough health assessment, our primary care team will visit you regularly to provide your medical care and coordinate referrals to any specialty care that you may need. Our goal is to keep you healthy and prevent unnecessary and disruptive trips to the emergency department or hospital. We’ll work closely with the skilled nursing facility staff, you and your family to make sure that you are an active participant and decision maker in your care.
