TIHP Membership Has Its Privileges
As a valued member of TIHP, you’ll receive personalized care from our team of highly qualified physicians, mid-level providers (nurse practitioners or physician assistants) and nurses. They’ll work together to ensure that potential health problems are prevented or caught early and carefully coordinate your medical care.

Every year, Medicare evaluates plans based on a 5-star rating system.

Who can join an I-SNP?
You may qualify for a TIHP I-SNP if:
- You have both Medicare Part A and Part B.
- You’ve lived (or plan to live) in a contracted nursing home or skilled nursing facility for longer than ninety (90) days.
Who can join an IE-SNP?
You may qualify for a TIHP IE-SNP if:
- You live at home and/or in a contracted assisted living facility.
- Texas has certified that you need the type of care that is usually provided in a nursing home.

After providing you with a thorough health assessment, our primary care team will visit you regularly to provide your medical care and coordinate referrals to any specialty care that you may need. Our goal is to keep you healthy and prevent unnecessary and disruptive trips to the emergency department or hospital. We’ll work closely with you and your family to make sure that you are an active participant and decision-maker in your care.
